Cervical Mucus test

The status of the cervix can also be tested following the postcoital test to evaluate it according to the following factors:
  • Amount of mucus.
  • Quality of spinnbarkeit (the stringy elastic character of cervical mucus during the ovulatory period).
  • Degree of opening.
  • Presence of ferning (the patterned appearance of the mucus on the slide, see illustration below).
  • Clarity of mucus versus the presence of cellular debris or leukocytes.

The following table describes how the various factors in the cervical mucus evaluation are combined into a single score. Each of the four categories receives a score from 0 to 3 that is totalled to a final score ranging between 0 (negative) to 12 (excellent).

Cervical scoring system
Parameter0123
Amount of mucusNoneScant: a small amount of mucus can be drawn from the cervical canalDribble: a glistening drip of mucus seen in the external os; mucus easily drawn from the cervical canalCascade: abundant mucus pouring out of the external os
SpinnbarkeitNoneSlight: uninterrupted mucus thread may be drawn approximately ¼ of the distance between the external and the vulvaModerate: uninterrupted mucus thread may be drawn approximately ½ of the distance between the external os and the vulvaPronounced: uninterrupted mucus thread may be drawn for the whole distance between the external os
FerningNone - amorphous mucusLinear: fine linear ferning seen in a few spots, no side branchingPartial: good ferning with side branches in parts of the slide, linear ferning or amorphous mucus in other partsComplete: full ferning of the whole preparation
CervixClosed: mucosa pale pink, the external os hardly admits a thin applicator Partially open: mucosa pink, the cervical canal easily penetrable by an applicatorGaping: mucosa hyperaemic, the external os distended
